Released in Australia, New Zealand, and Japan in 1999 before reaching the US and UK in January 2000, "Otherside" gave Red Hot Chili Peppers their fifth number one on the Billboard Modern Rock Tracks chart. It stayed at the top for 13 consecutive weeks, one of the longest runs in the chart's history, and peaked at number 14 on the US Billboard Hot 100. In Iceland it became the second single from Californication to reach number one, while in New Zealand it charted at number five.

Anthony Kiedis wrote the song about Hillel Slovak, the band's former guitarist, who died of a heroin overdose in 1988. The lyrics confront the battles addicts have with their prior addictions. The single was the third taken from the band's seventh studio album.

Jonathan Dayton and Valerie Faris directed the music video in a black-and-white Gothic style influenced by German Expressionist art, particularly Robert Wiene's The Cabinet of Dr. Caligari. Dayton noted that Caligari was both an inspiration and something to work around, because other videos had completely ripped it off. Faris said they were inspired more by paintings than by films, drawing on futurist artists from the 1930s and the illustrations of the surrealists and cubists. In the video, Kiedis appears with short platinum hair in a castle tower, while John Frusciante plays a rope down a corridor as if it were a guitar. Flea plays high voltage wires as a bass guitar, and Chad Smith uses a rotating medieval clock as his drum kit.
