Josh Homme wrote "No One Knows" with Mark Lanegan, though the song existed well before Queens of the Stone Age began recording Songs for the Deaf. Homme said the band is patient with music. Two songs on the record, "God Is in the Radio" and "No One Knows", were over five years old. Recording started in March 2002 at three studios across California.

Released on November 26, 2002, as the first single from the album, it reached number one on the US Modern Rock chart, number five on Mainstream Rock and number 51 on the Hot 100, all career highs for the band. It was also the band's only single to top the US Alternative charts. Triple J listeners voted it number one in the Hottest 100 of 2002, and it earned a Grammy nomination for Best Hard Rock Performance, losing to Foo Fighters' "All My Life", credited to Dave Grohl, who had been filling in as drummer for Queens of the Stone Age.

The video was shot over two months in June and July. Homme chose Michel Gondry's style because the band were big fans of Björk. A deer attacks three band members after they hit it with their truck, then mounts their heads on a wall. Mark Ronson later covered the song as a B-side with Domino Kirke on vocals, and it charted in the UK on its own.

No One Knows runs at 170 BPM, the tempo of drum and bass and jungle. In classical terms it sits in vivace territory. A bar of 4/4 lasts 1.41 seconds, and a quarter-note delay is 353 ms. It is in C major, Camelot 8B, which shares its notes with A minor and mixes cleanly with F major and G major.
